Overview of the Incident: (General description of the incident, to include as much detail as is deemed necessary)

Much of what I know comes from what others have told me about the incident, my personal witness to the manslaughter of my colleague at the hands of Robotics is short and shiny for they carted her off before I could question them further. On this particular shift I peeked into the window of the Robotics lab and noticed in horror the deceased body of the now passed Mother, I inquired as to what had happen but was ignored at first so I banged on the window and demanded an answer. They merely replied she had passed away during Surgery for repairs on her arms and carted her off. Shocked, I said a requiem for her but merely thought it had been a genuine accident, after arriving on the ODIN however I asked around and was informed that this 'accident' was entirely avoidable and was as a result of rancid neglect. Apparently, advanced surgery originally devised by CMO Gonzales was left in the hands of two amateur Roboticists who should of refused the Mother that kind of work, not only that but during this advanced surgery, organic arms were left out on the table to rot. They then attempted to attach said rotten organic arms to her which promptly killed her. This is, in my view, manslaughter at best, murder at worst. The very fact that a scientist and roboticist opted to perform advanced surgery on an OLD WOMAN IN HER 90s and then left arms out to ROT strikes me as far too stupid to be an accident, only advanced stupidity could of resulted in this disgrace so I am leaning more towards a murder plot. Whatever the case these men ought to be arrested and charged. This event is doubly shocking due to the Churches policy on cloning, there is no second chance for her.



Did you report it to a Head of Department or IAA? If so, who?: Sadly this was not reported due to my first assumption of it being a genuine accident. (I also had to leave OOCly)

Actions taken: Nothing done on the shift due to external factors and too little information. Afterwards I spoke with my fellow colleagues about the matter and noticed this sinister plot.
