Reporting Personnel: Morgan Renalt

Rank of Reporting Personnel: Surgeon

Personnel Involved: Chemist Sean Cromwell

Time of Incident:

Real time: ~2 PM CST

Location of Incident: Medical Bay

Nature of Incident: []Workplace Hazard []Accident/Injury []Destruction of Property [x]Neglect of Duty []Harassment []Assault []Misconduct []Other _____

Overview of the Incident: At the beginning of the shift, I began my usual routine of preparing medbay for the shift. Over the comms system, Dr. Cromwell began talking about using unstable mutagen to attempt to grow his hair. As he had already administered the treatment, I left it alone. However, soon after, I noticed that he was neglecting his duties in order to continue experimenting on his hair. I informed him that we should have a stock of alkysine, and a beaker of either cryoxadone or cloneoxadone. He agreed and went silent for a brief period, before he informing me, using coarse language, that it was too hard. I explained that it was his job, but he continued to ignore me. Later in the shift, I noticed that he had begun vomiting after administering more radium to himself. I treated him for radiation injuries, but he continued to neglect his duty and experiment with radiation therapy. Finally, after a few reminders and gentle prods, he made a beaker of a liquid that may or may not have been alkysine, as it was not labelled, and brought it to me rather than putting it in the refrigerated storage unit. I deposited it safely, where it was left unlabeled for the remainder of the shift. In addition, he continued to experiment with radiation therapy and make messes all over his lab and the staff bar. At my last check, he had still managed to not synthesize a properly labeled, beneficial compound, while simultaneously managing to clear out the majority of the reserve medicines in the medicine storage room. He did eventually give up his attempts at radiation therapy, and began acting in a civil manner, but he was profoundly unhelpful for the entire shift. Very unprofessional.

Did you report it to a Head of Staff or IAA? If so, who?: None were available at the time of the incidents.
