If other, replace line and specify.) Overview of the Incident: Phyliss and myself (Styles) agreed to spar to pass some time. During this spar I fractured one of her head bones (I don't know which.) With hindsight it's clear I used far too much force in my strikes. I'm attaching my statement on my mens rea during the incident in the form of a confession. As a Security Officer, there remains an expectation that you can be, and should be, capable of determining as these situations occur, proper sentencing. In this instance, Human Resources believes that this is not a suitable report for the purposes of Human Resources, and is best left to be handled by the Security Department.
