Diggey Posted March 1, 2022 Posted March 1, 2022 (edited) Reporting Personnel: Keira Pfeifer Job Title of Reporting Personnel: Service manager Game ID: round before cgJ-aqEk Personnel Involved: Anya Orlova, Chef , Offender - Isskssi Issana, Cook, Witness/Victim - Keira Pfeifer, Service Manager, Witness Secondary Witnesses: Nariah Sewell, Head of Security, Was approached about the Documents filed about the report and wether or not she had to acknowledge her own Misconduct if the complaint is not from a Head of Staff Time of Incident: 00:45 Real Time: 3:05 GMT+1 01.03.2022 Location of Incident: SCCV Horizon Kitchen/Hydroponics Nature of Incident: [X] - Workplace Hazard [ ] - Accident/Injury [X] - Destruction of Property [X] - Neglect of Duty [ ] - Harassment [ ] - Assault [X] - Misconduct Overview of the Incident: Unauthorized unprompted modification of the Horizon that resulted in the depressurisation of a Department and the endangerment of Crew at around 00:15 Anya Orlova had been unfrozen from Cryogenic storage, I was coincidentally just finishing up with cleaning up a biohazard in the Medbay and was as such returning from the Custodial closet. When I attempted to exit the Elevator the doors shut in front of me, someone else, Anya Orlova, had called the elevator up, we entered together, I attempted Smalltalk, Anya was dismissive. I set the Elevator to our Level, Level 2, and exited, Anya stood in the elevator looking at me, told me she hit the wrong button, then went to a different level, not the level she was assigned to. I had a call from the Warden, after doing the rounds I returned to the Bar and Lounge, manning it due to our lack of a Bartender to offer recreational options to the crew, I saw Anya going the same way as me, for no reason she dipped into the Public garden, clearly trying to avoid me, I decided not to press the issue in order to make the shift as amenable as possible for her. The other cook, Miss Issana, came ,from the Living quarters, to work in the Kitchen, while she was passing the bar I informed her firmly to contact me either through comms or my PDA or via fax to my office (an unclaimed Office I had decided to set up shop in) if there was anything that needed doing, any supplies required or anything else I could do for them in the kitchen. Some time later the Warden contacted me again about Reishi, not beeing a Hydroponocist, I decided to head into Botany in order to check the Botanical records on the plant, this is where I noticed that there have been significant changes to the Hydroponics, including the removal of barriers that would normally block the airflow between departments. This includes, I believe, an Automatic airlock. I saw Miss Issana standing there so I started reprimanding her about the Alterations and how I specifically notified her of my role on this ship, the ability to issue Guest Passes, grow plants myself, or authorize alterations to the workplace if I ,as the Manager, deem them reasonable, she seemed clearly unconfortable, like as if she was taking the blame for something she had not done. She did not correct me however. As I was filing the necessary paperwork to remedy this issue and Absolve Issana of any Crimes she may become implicit in, through the alteration of SCC property, a Meteor hit the bar, creating a hole in the kitchen, and in conclusion emptying the Botanical Department, within which was a Cow, unsupervised, which likely died as a result. My fears had come true immediatly. I made a copy of all necessary Documentation to Absolve the kitchen After beeing rescued from my office (which had also been depressurised) I handed Miss Orlova a copy of the documents, warning her that this had been her fault, as well as a Guest Pass to authorize access to the Botanical department, I then headed to my crew quarters to blow off some steam after my entire Department had been effectively destroyed by the actions of what I believe to be one person. I later learned that these documents have been discarded and my authority in issuing these was questioned. The guest Pass was also passed off to a different person, not the person it was issued to, which I also believe to be a crime. Which, you know- Submitted Evidence: Attached are copies of the unfinished unsigned Documents the originals of which have not been handed to their respective Departments for handling and filing. These were located in the Unassigned Office above the Horizon Chapel, manually labeled "Service Manager" with a Hand Labeler Would you like to be personally interviewed?: [X] - Yes [ ] - No Did you report it to a Head of Staff or a superior? If so, who? If not, why?: I chose not to submit the final document at the time of the incident as a Disciplinary measure, as a Mercy. I later learned that the Documentation was entirely neglected and the Guest Pass Issued to Miss Orlova was handed off to Miss Issana instead. Actions taken: I had informed the Victim and the Offender that all they had to do to gain access or Recources was to ask me, A thing I had explicitly informed the Victim about and attempted to inform the Offender about who purposefully attempted to avoid me, going so far as going into an elevator, and changing floors in an attempt to avoid contact. I had informed both over the Radio an additional time. After the damage had been done I attempted to file the necessary paperwork to retroactively absolve the offender of their crimes in an attempt to keep the Department operational without Security intervention. Additional Notes: This attempt has not only been ignored but actively been avoided, therefore making the crimes committed unabsolved and official. They did NOT have Access they did NOT have permission to enter the other Department they did NOT have permission to make the alterations, They did NOT use their Guest pass to circumvent this they did NOT talk to me to resolve the Incident report inside the Department and I am therefore forced to take this up the Line. Miss Orlova is trying her hardest to not comply with corporate procedure and to do as much damage to the Horizon as possible. It is not only the duty of the departments to leave things as they found it to guarantee a safe turnover to the next shift and an easy start to procedure for following colleagues, it is also their duty to not Depressurize unrelated departments in the pursuit of pursuing their vague job description over actually doing their actual job and serving the Crew like she was supposed to.
