Time of Incident: Roughly 00:00 to 1:30

Real Time: 9:30pm EST, ish
Location of Incident: Medical Bay

Nature of Incident:
[ ] - Workplace Hazard
[ ] - Accident/Injury
[ ] - Destruction of Property
[ ] - Neglect of Duty
[ ] - Harassment
[ ] - Assault
[x] - Misconduct
[x] - Other: Repeated inability to concede to comfort of other crewmembers and follow orders from a superior

Overview of the Incident: Dr. Donitz repeatedly insists upon using nicknames for personnel in the medical bay, which so long as other staff have no trouble with, I was otherwise content. However, prior to this particular shift, I mentioned to him I did not want a nickname and would prefer to be addressed by simply "Dr. Kassaka" or just "Kassaka". I also stressed that I did not want to be called by my first name, as he suggested some variety of nickname based on it. As I am sure staff are aware, it is offensive to refer to Sinta'Unathi by their personal name if one is not close, akin to establishing a far too personal relationship with someone too quickly. This was elucidated to Dr. Donitz previously, as he has had similar incidents with another Sinta coworker.

However, this shift, Dr. Donitz chose to address me as "Sralza" while speaking to me about an emergency. While he did not say it over communications, fig. 1 shows my response, and his response thusly. I decided to let it pass as I was later busy with directing a few minor responses. Later, however, I asked him to stage in the OR as there was a workplace accident in xenobotany, and he responded with "as you wish, Sralza". Fig. 2 was my response.

After the workplace accident incident had passed, he addressed me on communications with a shortened version of my first name in fig. 3. I admit here is where I lost composure and, as the images of the archived communications say, I did swear and call him an "insolent cad". I regret my loss of composure immensely. As his immediate response was to try and push back against me, I spoke to Captain Helena Artigas and requested that she stop him from continuing to act disrespectfully. Artigas' message to Dr. Donitz is as seen in Fig. 4. Dr. Donitz proceeded to remain silent, as requested, for the remainder of the shift.

Near the end of the shift, I took Dr. Donitz into my office and told him in no uncertain terms that it was offensive for him to continue to use my first name, and the fact he had continuously ignored my requests to stop doing so began to cross the line into deliberate xenophobia because he refused to acquiesce to my cultural norms. I also told him I did not want a nickname, and it was not a debate. He continued to try and argue against me, refusing to agree to my terms of calling me "Dr. Kassaka". I decided to send him out of my office and begin work on this incident report, as his defiance does not befit how he speaks to both a direct superior and the captain of the ship.

Would you like to be personally interviewed?:
[x] - Yes
[ ] - No

Did you report it to a Head of Staff or a superior? If so, who? If not, why?: I am a head of staff, and I requested aid from Captain Artigas.
Actions taken: Dr. Donitz was verbally reprimanded by Captain Artigas (fig. 4), and near the end of the shift I took him aside to explain why his behaviour was unacceptable. As he continued to combat the request of a direct superior, I made the decision to file this report.

Additional Notes: N/A
