Nature of Incident: [X] - Workplace Hazard [X] - Accident/Injury [ ] - Destruction of Property [X] - Neglect of Duty [X] - Harassment [ ] - Assault [X] - Misconduct [X] - Other - Violation of Regulations Overview of the Incident: Part way through the shift, there was a sudden call to evacuate the cafeteria, it turns out the room had been breached by space dust, which lead to several crewmembers being injured by the decompression. It was found out that the ship's shields hadn't been up while flying through a dust cloud, which lead to the breach. As the shift continued, they heard that the captain had spoken with the crewman piloting at the time and 'handled it internally' (rather then security handling the charge and effectly pardoning the pilot of their negligence) and refused to let this one and Samantha speak with the crewman. The captain was rather rude to them at that time. As the shift was ending they heard that the chief engineer was agreeing with the captain about the situation being handled internally, and the captain was again rather rude to them on the situation. They were speaking to Cohle about the incident, and he spoke to the captain on the radio, in which the captain claimed they would be happy to allow HR to mediate on the questioning of the chain of command. They did have a chance to take to Leon at shift-end, and he told them that the captain had ordered him to fly through the dust with shields down. Melariara Posted October 26 Posted October 26 Hello. I will be binning this for OOC reasons but I will explain it. In this particular case, the pilot was new to the role and being taught how to pilot, ICly, by the captain and, later on, another BC. The shields were not up due to an apprentice engineer also learning from the CE that shift. CCIA and staff as a whole do not want to discourage people from learning new roles, and we are sure that the pilot learned a lot from this shift. At the same time, we do commend security for reporting this in an IR, as if this was a repeat offense, this would be the appropriate action to take and the correct way to follow SOP. 1
