Overview of the Incident: 

 

In the late hours of the 20th of May, 2468, an Outer Eyes agent was captured by the Internal Security Department following an attack upon the SCCV Horizon. This agent, a shell who called themself “Zeta Mu,” was taken into custody and interrogated by the investigators under the command of Head of Security Forbearance; however, due to the scheduled shift change, the interrogation was unable to be completed. As the prisoner’s information was considered of high value to the S.C.C., the decision was made to hold them for interrogation for a second shift. The shift change was completed without incident, and the prisoner was placed into the custody of Head of Security Seok. 

It was in this second shift that the true incident occurred. The investigations team took the beginning of the second shift to collect their thoughts and prepare for the continued interrogation, during which time, the prisoner was placed in the communal area of the Horizon’s brig. At the time, there was no Warden on duty, but there was Dr. Seok and three officers, whom I have named above. After informing Doctor Seok of the importance of this prisoner and briefly speaking with the prisoner myself, I trusted the Internal Security Department to handle the matter as I continued my rounds through the other departments. It was not long after I left security and while I was speaking with the scientist on duty, Z.I. Akatsiya, that I heard Bridge Crewman Perlaylna’Shirani alert me over the command channel that an escape pod had been detected leaving our sensor range. As no launch of an escape pod was authorized or reported, the helm undertook pursuit maneuvers. As they did so, security checked the brig, and found the prisoner missing, as well as a hole cut into the floor of the communal restroom (pictured below), which led directly into the escape pod. It is the belief of the security forces that the prisoner used a cutting tool of the like found in emergency toolboxes to make their escape. This would have required the prisoner to be left unsecured, unsupervised, and in reach of such equipment, despite three officers being on staff and capable of preventing this occurance.

At this time, the escape pod was tracked to a nearby exoplanet, and a ground team under the command of Doctor Seok was dispatched to apprehend the prisoner. The team arrived too late to catch the Outer Eyes agents, as indicated by notes found on the site and evidence of a recent shuttle launch, corroborated by a warp signature leaving the sector, which was detected by the bridge. The ground team was recalled, and by the time they returned, the shift was nearing its end, preventing meaningful investigation.

Did you report it to a Head of Staff or a superior? If so, who? If not, why? I was made aware of this event by Bridge Crewman Perlyalna'Shirani. At the time, it was not forwarded to central command due to my focus on recapturing the escapee.

Actions taken: Doctor Seok was ordered by myself to begin an investigation into the events which lead to the escape of the prisoner, however, due to the time constraints of the shift, no meaningful progress was made.

 

Additional Notes: 

 

A prisoner escaping from the custody of the Internal Security Department is an egregious problem under any circumstances, but this loss of this prisoner is an extreme case as, not only did they have crucial information to the operation of the vessel, but they were responsible for the death of Officer Impukane in the prior shift. The loss of Officer Impukane weighs heavily on the minds of the crew, and the fact that his killer could not face justice has had a significant effect on the morale of the crew and the trust in the Internal Security Department. (I refer the investigating agents to the Relay used by the crew of the Horizon, between 01:13 and 16:52 ship-time on the 21st of May.)

It is worth noting that Doctor Seok told me that she believed this escape was preplanned, and we considered the possibility that the escape was allowed intentionally by an Outer Eyes sympathizer or infiltrator within the security department, who may have planted the cutting tool in advance or otherwise assisted in the escape. 

Furthermore, it is rumored that several such escapes of Outer Eyes agents have occurred in the recent weeks. I have only been personally present for this incident, and have been unable to confirm this hearsay with records due to an NT-net error in the residential wing. If true, however, this is a grievous and ongoing fault of the Internal Security Department, and our policies for the containment of high-value prisoners must be reexamined on a systemic level.
