Reporting Personnel: Girard Smark

Rank of Reporting Personnel: Scientist

Personnel Involved: Girard Smark-Scientist, Karima Mo'Taki-Scientist, Serenity Neferet-Research Director

Time of Incident:

Location of Incident: Miscellaneous Research, Science wing

Nature of Incident: [x]Workplace Hazard []Accident/Injury []Destruction of Property [X]Neglect of Duty []Harassment []Assault []Misconduct []Other _____

Overview of the Incident: Director Serenety Neferet requested that me and Scientist Karima Mo'Taki test experimental weapons on proto-humans. Karima Mo'Taki requested not to perform this testing, which Director Neferet allowed, leaving me to test alone. I performed weaponry testing as requested, and upon finishing tasted, I noticed I had accumulated a buildup of toxins in my system, most likely due to a uranium leak in the experimental weapon. I relayed this information to Karima Mo'Taki, who then questioned Director Neferet. Director Neferent informed Karima Mo'Taki that she was aware of the possibility of radiation leaks. However, Director Neferet at no point informed me of the possible danger of testing this weapon, and left me open to potentially deadly workplace conditions.

Did you report it to a Head of Staff or IAA? If so, who?:No.

To whom it may concern;

Greetings. She is Karima Mo'Taki, and she would like to clarify the report as a witness. After producing protopistols (ranged weaponry), Director Neferet was about to deliver them to the security department. Herself, Karima Mo'Taki expressed concern for a component in the gun, which was susceptible to leaks and thus, emitting a harmful radiation. Serenity then stated that if such was the case, it would go through testings.

She did not wish to perform the weapons tests upon protohumans, so it was conducted by Mr. Smark. Upon checking up on him, he noted he received radiation damage, just as predicted. He was taken to medical. If the weaponry had leaked in security's possession, it may have caused terrible consequences.
