Nature of Incident: [x]Workplace Hazard [x]Accident/Injury []Destruction of Property []Neglect of Duty []Harassment []Assault []Misconduct [x]Other: Reckless endangerment; Failure to execute an order; Refusal of demotion. Overview of the Incident: Some time into the shift, after power had been established, a pneumatic cannon was commissioned for prototype testing and material integrity evaluation. Once built, it was loaded with steel spoons and testing began. The first test was simple accuracy; Monique Byrd volunteered for this test, and requested I shoot her from appx. 12 meters range. I did not realize the cannon was set to full power, and as a result, Monique sustained a light injury which was quick to be treated. Next, I volunteered for the same test at a lower power level. As Byrd prepared to initialize the test, she was unwittingly approached from behind by Fay Thorley, who attempted, without warning, to forcefully take the weapon from Byrd. The resulting struggle ended in a reflexive misfire, which caused bodily harm to Samara Kaytam, within appx. 1 meter of the weapon at the time. Her hand was broken, and I believe she was taken into surgery for treatment. After this, I contracted a virus and had to report to medbay. While this happened, several events developed of which I was not, at the time, aware. Later, I was told(without any proof, may the record note,) that several engineers had been slandering and harassing Thorley. I never saw substantial evidence of this. After the event, I filed paperwork for the demotion of Fay Thorley. I faxed a copy to the Head of Personnel and gave another to Fay. She outright refused the demotion. I attempted to have her detained several times, but each time, a matter elsewhere on the station seemed to come up and she continued to dodge the demotion, claiming it to be unlawful(also without proof). Both Fay and myself spoke with the IAA, and I also spoke with the Captain. When I finally could get a security officer(Carlin Hague) to escort Fay to the HoP's office, she ran from the officer and to the IAA office. It was at around this time the transfer shuttle was called. Due to time constraints, no punishment was carried out, though I was told several other engineers had also committed crimes. Most notably, the IAA told me the following: - "Manfred Hayden would be charged with vandalisim, threat of bodily harm and slander" - "Monique would be vandalisim and slander" - "Xullie, slander" ... "And minor assualt" I was accused of having threatened/slandered Fay over comms, and I have pulled the common and engineering channel Telecom logs from the entire shift. Unfortunately, these are rather large, but perhaps she can point out exactly where I did this. I said a total of ten things over common, and none were threatening. I said a significant amount more on engineering(being the CE), and despite my fairly profane nature at times, I believe wholeheartedly that I said nothing threatening or slanderous about Fay over either channel.
