Overview of the Incident:

At around 12:30 Medical was prepping to repair the second OR, we had already ordered a surgery crate and the captain arrived, where he had told us that engineering will be shortly arriving to refurbish the second OR and that their deadline was by 12:50.

Engineering arrived by 12:40, Engineers Monique and Woodrow arrived without the needed equipment, and they had only started working after 12:45.

The Chief Engineer arrived by 12:55, moved into the first OR, relabeling it OR 2 via his blueprints and then vice versa with the second OR to bypass the deadline.

After the great feat of incompetence and neglect of duty, the chief engineer proceeded to pretend the second OR was repaired and that now the first one needed to be repaired

by around 1:10 the captain came to inspect the second OR, which I had demonstrated to be in nonoperational condition with missing floor tiles, missing old machinery and biological remains in the area.

1:25 Medical was damaged in a security incident, and required repairs, with the damage ranging between wall plating to cryogenic atmospheric pipes. only half the damage was properly fixed, leaving a broken disposal pipe throwing bio hazardous leftovers on the medical floor, contaminating it's sanitation.


Engineering was very ineffective for most of the shift and most likely due to lack of coordination. with engineers coming for repairs and leaving due to lack of cooperation from their peers.


 



Did you report it to a Head of Department or IAA? If so, who?: The captain, Paul Jonson, but they were too busy with other incidents at the time.
