Time of Incident: 2459-12-06 , around 06:40 Station time Real time: around 3 am, December 6, 2017 Location of Incident: Mining airlock/mining storage/mining smelter Nature of Incident: [x]Workplace Hazard []Accident/Injury [x]Destruction of Property [x]Neglect of Duty []Harassment []Assault [x]Misconduct []Other _____ (Place an x in the box that applies. If other, replace line and specify.) Overview of the Incident: In a nutshell, vaurca miner Zakk was impatient to wait for the airlock to cycle. So he destroyed the airlock's reinforced glass and grille. Not happy with that, he also welded the safety shutters that dropped, after he went through them. Which meant no one else could use the airlock, since he had effectively broken the air insertion process, and neither use the welded shutters of the already damaged windows. This meant that I, Ilya Likoyan, was stuck outside for over 20 minutes with a dwindling air supply with no method for getting in other than breaking the remaining window. This also meant that no other miner could either enter nor exit, making the entire mining department unable to do their job, as well as endangering them due to not being able to get in or out safely. As a cherry on top, Zakk parked his Ripley in front of the mining stacker conveyor belt, thus keeping minerals from being distributed to the station, fucking everyone in the process. He also DNA-locked it, so no one else could move it to a proper location. Not only was his conduct unprofessional, but he did everything ON PURPOSE, disregarding all the safety hazards and productivity of others. This type of "professional" shouldn't even be allowed to work near others. Did you report it to a Head of Department or IAA? If so, who?: Yes; Captain Seth Gramaton and Head of Personnel Amani Al-Kandari ((however she was tator'd that round)) Actions taken: Complaint form made and delivered to Captain. Captain forwarded the complaint to Head of Personnel Amani. No further action seems to have been taken. Link to comment
