Overview of the Incident:
Due to a workplace accident that occurred within the operations department late in the shift, he found himself severely injured, bleeding and unable to properly breath. He was taken over to medical by Tulkir, as he was unable to move properly under the pain he was in, who then called for doctors to come and help him. Amy Veir, the surgeon, came quickly alongside the paramedic and took himself inside of medical on a bed.

Though he was in intense pain, he is able to recall several strange things that happened while he was inside. He was taken inside and examined by the surgeon using a handheld scanner, who then took him off the bed and demanded something of him. He could not recall exactly what, as his vision was hard and he could not tell what was exactly happening around him. He believes he was then taken over to surgery, where the same command was demanded of him. He believes it was a call to put on a tank and mask- he did not have a tank on him, and his mask was hanging loosely around his neck from an earlier unrelated incident. Things became incomprehensible to him past this point.

When he next came to be aware, he was sitting on the surgery table. His body was screaming at him, and he could not stop himself from screaming in pain. He was taken back to the main room, his chest hurting so much, and was shoved into a body scanner roughly by the surgeon. He believes this was the first time she actually fully scanned him. He does not believe he was scanned before he had surgery. He was rolled all over the main room, shoved into one thing after another, all while he was in intense pain and not given anything to help with it. At some point, he was taken back to the surgery room for something- he believes his chest wound may not have been fully closed, but the pain made it hard to remember. He does not remember what happened when he was in the surgery room.

He came back to in the main room once more, still in pain. The surgeon seemed to finally realize he had been in pain the whole time, and gave him something for it. He will not lie- he was extremely angry at the surgeon at the time, screaming at them. They had left him in intense pain and had not bothered to get full scan before he was put through surgery, and was treated crudely throughout the process in what parts he could remember. He was vaguely aware of joke being made at his expense at some point, though it was hard to tell. After he was left in the main room from the second return, the surgeon quickly left the shift and went down to residential.

He has been deeply shaken by this whole experience. He feels like the surgeon neglected to do their duties properly, and left him in excruciating pain while performing surgery with lacking information. This is completely unacceptable and he would like something done about it.

Submitted Evidence: Due to ongoing security issues, nobody was around to take pictures or record things to his knowledge. Those he stated as witnesses can give testimony as to what happened.

Would you like to be personally interviewed?
[X] - Yes
[ ] - No

Did you report it to a Head of Staff or a superior? If so, who? If not, why?: There was no command on shift, and anyone available that could have helped him were busy with unrelated security issues.
Actions taken: See above. Nobody was able to do anything due to unrelated security issues and the fact she left.

Additional Notes: He has been warned by colleagues that he should spend a few days recovering, and one of his managers came to him after shift to advise him his shifts had been reduced. He hates to feel useless in this trying expedition, and he feels the surgeon has played a role in taking him out of service. His coworkers advised him to file this, and he believes it is the right course of action,
