Arrow768 Posted December 27, 2017 Posted December 27, 2017 Please find the Incident Report filed by Leslie McAllister attached below. I could not address the incident report during the shift as I was only made aware of it minutes before the crew transfer shuttle has been announced. with best regards, Paul Jonson, Captain, NSS Aurora. Reporting Personnel: Leslie McAllister Rank of Reporting Personnel: Psychologist Game ID: bR8-dxh6 Personnel Involved: - Johnny Styles, Security Officer (Offender) - Vincent Burns, Security Officer (Offender) - Leslie McAllister, Psychologist (Victim) Time of Incident: Real time: 02:00 GMT+1 Location of Incident: Nature of Incident: []Workplace Hazard[]Accident/Injury[]Destruction of Property[]Neglect of Duty[x]Harassment[x]Assault[x]Misconduct[]Other _____ (Place an x in the box that applies. If other, replace line and specify.) Overview of the Incident: At about 04:08, I (Leslie McAllister) stepped into my office for a smoke break. Upon opening up my lighter to light a cigarette, the cologne I was covered in ignited, and I was lit on fire. The injuries sustained were minor, but they caused me a great deal of distress, along with damage to my office. Two members of the security team answered my distress call-- Johnny Styles, and Vincent Burns, both of which promptly arrived to the scene. After my injuries were treated, the two began interrogating me regarding the incident. Mr. Styles' first comment was about how funny it was that Mr. /Burns/ came to talk to a burn victim, which set the tone for the rest of my interaction with the two. Mr. Burns accused me of being dumb for catching fire, while Mr. Styles continued to accuse me of lying. This culminated in him lighting up a cigarette and throwing it at my chest in an attempt to validate that it was in fact, cologne. This of course did not ignite me, as the cologne had burned out and dissipated by the time I've rolled around on the floor and suffered these injuries. The two immediately began insulting me-- Mr. Burns referred to me as a "real shit liar", whilst Mr. Styles accused me of having no style. When informed that I'll be taking the issue to Command, Mr. Burns told me to take it up my ass. Aside from their open and unacceptable hostility, the two have shown a complete lack of proficiency in their occupation by not only failing to conduct a proper investigation, but by risking my life by throwing cigarettes at me. Actions taken: Command Staff has been informed, but the incident could not be addressed until shortly before the arrival of the crew transfer
