Nature of Incident: [X] - Workplace Hazard [X] - Accident/Injury [X] - Destruction of Property [X] - Neglect of Duty [ ] - Harassment [X] - Assault [X] - Misconduct [ ] - Other _____ (Place an x in the box that applies. If other, replace line and specify.) Overview of the Incident: As I was rising from the residential lifts, I like heard an announcement about a rogue mech - Something that was extremely alarming. As I made myself known and was briefed by the Bridge Crew, mainly Asher and Marcus Zhao. I was informed that John Kodiak, a cargo technician with a habit of getting into some trouble, had began a rampage through the halls in a mixmatched exosuit. I heard him shout over the common channel - 'If you're going to falsely charge me, I'll give you a reason to charge me!'. Mr. Kodiak continuously declared that he had a right to self defense, and that if he was to be harmed he'd defend himself against any attacks. During his rampage, he damaged several walls, nearly crushed some crew - I don't know their names, broke several windows and airlocks. He ended up surrendering after Commander Tokash brought the PEAC out. The incident ended in the Grauwolf. Later into the shift, during his holding.. Mr. Kodiak assaulted the Head of Security repeatedly - so much so that he was placed in Isolation Cell B, where he only resisted further. This behavior is extremely unbecoming of a professional. Several peoples lives were put in danger by his reckless actions, and while no one was outright harmed, he showed the intention to do such. Submitted Evidence: Due to a separate issue that was handled, I was not able to gather any evidence or testimonies from the crew. Would you like to be personally interviewed?: [X] - Yes [ ] - No Did you report it to a Head of Staff or a superior? If so, who? If not, why?: I was the Captain on shift. Actions taken: I suspended his I.D and we held him under the appropriate charges. Additional Notes: I recommend an immediate arrest to be done to prevent him from having access to any more tools of destruction, until the investigation is complete.
