Overview of the Incident: 

As I boarded the station out of a transfer shuttle. I have went on my visits to each departments in person than checking on them via headset as the usual norms. That's when I received a disturbing news about Max Judge was in a state of despair. I had PDA'd Max Judge, called him out in common radio frequency and have also asked Security to see where Max Judge is. Officer Kayta Fahiira reported that he was in the bar, I was beginning to revert Amaya Stone's actions. I had pulse checked with Command Staffs and Security Staffs to undo Amaya Stone's actions so Max Judge could continue working, the both departments were in favor of reverting Amaya Stone's actions. Within the minute, I received an intel from the crew, the barkeep, and one another person that Max Judge is now deliberately attempting to commit suicide, I called for action and informed Max Judge I would be more than happy to revert every of Amaya Stone's actions.

While I did not see this, I was told that Doctor Stephen Yates was immediately on the scene and carried Max Judge away to Medical Department to attempt an immediate recovery. Max Judge did not make it, he had passed away. Stephen Yates reported that he has done everything he could. Stephen Yates mentioned that Max Judge had fused three chemicals together: potassium, water, and PMT(?) drug that would "literally nuke his heart". During all this time, I had begun investigation by asking Command staffs, Security staffs, Cargo staffs, and so forth. The information that they all have reported have been the same to me. Head of Personnel Amaya Stone had suspended Max Judge and revoked his payroll instead of reaching out to Max Judge for help when he asked for one. He was pushed away and was turned to doing drugs. Crime Scene Investigator Raold Kharmenko had visited the morgue in Medical Department, Raold Kharmenko, Stephen Yates, and I have concluded the official cause of death was an assisted suicide. There were syringes to be found at the bar crime scene, but no one knew for sure where it came from. Raold Kharmenko was not able to report in a timely manner about the syringes that Max Judge had used on himself. Later, a funeral session was hosted to honor Max Judge with Stephen Yates leading the funeral services and cremation. 

Submitted Evidence: A Situation Report contained the same information to be used for Incident Report. 

Did you report it to a Head of Staff or a superior? If so, who? If not, why?: Yes, I faxed a Situation Report at the shift's end. I am unable to discipline or deliver an action when a command staff is not present. 

Actions taken: None whatsoever.
